REGISTRATION FEES

REFUNDS
In the event a player quits before November 1 and wishes to receive a refund, the request must be made to the Registrar in writing (email accepted). All refunds will be pro-rated from the beginning of the season, being September, to the end of the season, being February.
REP B, WILD & COBRAS TRYOUTS
- All players participating in the Rep B, the Wild or the Cobras tryouts must be registered with the SIMHA.
- A $10.00 ONLY, non-refundable fee is payable to the St-Isidore MHA at registration. Early bird discount ends on August 1, 2021.
- You may NOT participate in any tryouts unless the registration is complete and fee is paid to the St-Isidore Registrar.
- Do not send additional registrations fees until you know on which team your child is officially registered for the year (after tryouts)
SHARED INFORMATION
The SIMHA occasionally shares information with our Competitive Team affiliates - registration lists and DOB. If you do not wish your information to be shared please indicate this on your registration form.
PROOF OF RESIDENCE
- Driver’s License for Father
- Driver’s License for Mother
- School registration at a school in or near the residence for the current year
- Proof of mail delivery by Canada Post at residence (Box numbers are not acceptable)
- Telephone bill for land line service at residence
- Cable or Internet bill for service at residence
- Utility bill for service at residence
- Lease agreement for residence in parent(s) name(s)
- Current property tax notice in parent(s) name(s)
- Homeowner/Renter’s insurance policy for residence
- Void cheque with residence address
- Vehicle registration with residence address.
- Any other documentation which supports your claim to residency at the address
PROOF OF AGE
HEO requires all participants to provide proof of age. Please submit a copy of your child's birth certificate or passport (health cards are not acceptable). You may submit a scanned copy or photo as proof of age during registration.
You need only provide proof of age once. The verification will be entered into their Hockey Canada Registry record.
