Volunteer Registration Gore Bay Bruins-Western Manitoulin MHA

NOTE: Space is limited and registration is first come first served. Teams will be capped at a maximum of 18 participants. Registrations received after a team is considered full (on or before the registration deadline of September 15, 2024) will be put on a waiting list and may not be guaranteed a spot. 

PAYMENT: payment by bank interac e-transfer preferred. cash or cheque also accepted. Registration will not be complete until full payment is received. E-Transfer: send e-transfer to [email protected] - if a password is required, please use hockey2024 - Please include player name(s) in the memo section of the e-transfer. SIBLING DISCOUNT: 10% Off for 2nd, 3rd, 4th... child registered from the same immediate family. Players should be registered at the same time for discount to apply. If you register each player separately, the system will not show any discounts. e-mail [email protected] if discount is not applied correctly.

LATE FEE: $100 late fee will apply in addition to regular registration fees after September 15, 2024 (U7 division excluded). Late registration requests will be considered on a case-by-case basis at the discretion of the WMMHA Board of Directors if player numbers permit.

RESPECT IN SPORT PARENT COURSE: If your child has never played organized hockey before, you will need to complete the NOHA Respect in Sport Parent course. There is a $12+HST fee to complete the course. This course can be paid for and completed after registration. If you have a younger child who is new to hockey but you completed the course for an older child previously, you just need to log in to the NOHA Respect in Sport Parent site and add your newest child to your account so you do not need to do the course again. REFUNDS: Refund requests must be made in writing. Refund requests will be reviewed on a case-by-case basis at the discretion of the WMMHA Board of Directors. 

WATER BOTTLES: Players are responsible for their own water bottle and their name must be clearly labelled.  

EQUIPMENT: I/We hereby for myself, heirs, executors, administrators and assigns, at the end of the season covered by this registration, agree to return all equipment (includes home and away jersey) provided by Western Manitoulin Minor Hockey Association, in good condition, and should we fail to do so we agree to reimburse Western Manitoulin Minor Hockey Association for the replacement cost of such equipment.

SIGNATURE & WAIVER: I/We hereby for myself, heirs, executors, administrators and assigns, acknowledge the authority of Hockey Canada, Ontario Hockey Federation, Northern Ontario Hockey Association, Manitoulin Minor Hockey Association and Western Manitoulin Minor Hockey Association and agree to carry out and abide by the Constitution, Bylaws, Code of Conduct and rules and regulations of those associations.

RELEASE: I/We hereby for myself, heirs, executors, administrators and assigns, remise, release and forever discharge Hockey Canada, Ontario Hockey Federation, Northern Ontario Hockey Association, Manitoulin Minor Hockey Association and Western Manitoulin Minor Hockey Association, its officers, or anyone acting on their behalf from any manner of litigation, damage, claims, or demands in law or equity which I may have or acquire by reason of personal injury, loss or damage to property, which may occur during or by reason of participation in the activities of Western Manitoulin Minor Hockey Association.
