Cochrane Minor Hockey Association (CMHA) Payment Terms and Conditions

PAYMENT DUE DATES

All payments must be paid in full by November 1st, 2024. No participant will be allowed on the ice until full payment is received after this date.

Payments can be made in four payments – August 1st; September 1st; October 1st and November 1st. PLEASE ADVISE CMHA by email if you will pay in 4 payments at [email protected]

All tryouts fee (non refundable) must be paid prior to September 1, 2023 to be included on the tryout lists. TRY OUT FEE MUST BE PAID TO BE ELIGIBLE TO TAKE PART IN ANY REP TEAM TRY OUT.

FUNDRAISING

Mandatory for all players to sell 8 Cash calendars, this will go to toward are fundraising initiative. The calendars will have to be paid upfront at time of registration $200,00 once you sell all 8 you keep the money and return the ticket stubs to CMHA. There will be no rebate for calendar sales. 

VOLUNTEER HOURS

CMHA have applied a $200 volunteer fee. Parents or guardian are expected to complete 12 hours of volunteer time per family in the canteen or selling 50/50 during a CMHA tournament. Once hours are complete, CMHA will reimburse $200 fee. 

PAYMENT OPTIONS

I agree that full payment will be made by cash, cheque or e-Transfer.

Cash Payment: Please contact Caroline Gratton at [email protected] to make arrangement.

Cheque Payments: Please make out cheque to CMHA and drop off at Tim Horton Event Center in the mailbox beside office. 

e-Transfer: Please email to [email protected]. Make password "hockey". Please include participant's name in the memo/note section of the e-Transfer.

REFUND

I agree to pay CMHA all fees required for basic registration as well as any further fees as determined by any Representative or Select Hockey Program as required. I agree that there may be circumstances when a refund will be requested.

I agree that any request for refund will be made after reviewing the Refund Policy upon request from CMHA and filling out the applicable form for any refund request.
