Payment:

Payment for hockey registration must be made in full by registration fee dates posted.  Example, if you register before July 31, you must pay or request special arrangements by July 31 to secure that registration fee amount.  For questions about payment or setting up an arrangement, please contact Tiffany Robbins.  Payment may be made by:

• e-transfer - please send to [email protected] and use password Hockey
• cheque- If sending a cheque, please mail to BPMHA at Box 237 Lion's Head, ON N0H 1W0.

I agree:

• to provide BPMHA with a copy of my child(ren)'s birth certificate if they are a first time player(s). 
• to take the RIS Parent Program Certificate Number if I am registering with the association for the first time, or have not yet completed it.
• to pay the tryout fee, if my player(s) are planning to tryout for rep at $50/player - must be paid in advance of tryouts.  Tryout timing will be announced in September for U11, U13, U15 and U18 only.
• to provide volunteer support to assist our association and my child's team.
• that all rules and regulations will be abided by, by both the participants and their parents/legal guardian and their families.
• that all associated family members agree to abide by the terms noted above as well as the terms set by the BPMHA Rules of Operation, the constitution, any special provisions the playing sport regulated by the Province of Ontario and/or OMHA and the BPMHA code of conduct governing the association under which applicants shall participate in Bruce Peninsula Minor Hockey Association.

I understand:

• my child's image and/or name may occasionally be provided to the local media and or published on the BPMHA affiliated websites. If I do not give permission for videotaping or photography, I will notify the BPMHA Board of Directors immediately.
• that BPMHA or their sponsors assume no responsibility for injury or accident, except those that may be covered by insurance, as a result of such participation in, or travelling to or from such participation.
• that fundraising fees raised support the cost of referees, timekeepers, equipment used in practices, socks and other game day uniform requirements, player development, and more.
