SMHA Warriors Registration

Hockey is a team sport that involves a number of financial commitments on the part of each player/parent. Your fee commitment includes such items as governing fees, ice time and referees. You may choose to pay in one lump sum (due at time of registration) or in 7 equal payments due at the time of registration and on the first of the month starting October 1st - March 1st

1. Extra costs that are NOT included may be extra ice time, tournament fees and any personal expenses incurred.

2. Refunds - Stratford Minor Hockey Association will NOT issue refunds for any reason other than illness or injury that lasts more than 4 (four) consecutive weeks in duration. We realize that special situations may arise and these will be dealt with accordingly.

3. Willful Damages - Stratford Minor Hockey Association will NOT be responsible for any willful damage caused by ANY player/parent. This becomes the responsibility of each individual player/parent.

4. NSF - A $25.00 fee will be assessed for each NSF payment received. NSF payments that remain unpaid 2 weeks after notification will result in the player being prohibited from participating in games or practices until the debt has been repaid or until satisfactory arrangements have been made with the SMHA treasurer.

5. Non payment of any of the above fees may result in the suspension of the player.

6. For players who live outside of the City of Stratford limits, a $75 out of town fee is due October 1st as part of your initial registration payment.
