By submitting this registration form you have placed your child on the participation list for membership to South Simcoe Minor Hockey Association. Registration payments due dates will be posted on the South Simcoe Minor Hockey website. Failure to provide payment will result in removal from the participation list.

Please note any NSF charges incurred by South Simcoe Minor Hockey Association will be charged back to the player member. Requests for refunds will be reviewed and approved on a case by case decision.
