By submitting this registration form you have placed your child on the participation list for membership to Bradford West Gwillimbury Minor Hockey Assoication/South Simcoe Minor Hockey Association. Registration payments due dates will be posted on the Bradford Minor Hockey website/South Simcoe Minor Hockey website. Failure to provide payment will result in removal from the participation list.

Please note any NSF charges incurred by Bradford West Gwillimbury Minor Hockey Association/South Simcoe Minor Hockey Association will be charged back to the player member. Requests for refunds will be reviewed and approved on a case by case decision.
