 I/We give permission and consent to allow personal information [eg. group or individual photographs, name] of the above participant to be posted on www.sjpbaseball.ca as produced by the South Jasper Minor Baseball Association. I/We acknowledge that since participation in the website produced by the South Jasper Place Minor Baseball Association is voluntary, I/we will receive no financial compensation. I/we further agree that participation in the website confers upon me no right of ownership whatsoever. I/We release the South Jasper Place Minor Baseball Association, its contractor, employees and volunteers from liability for claims by me/us or any third party in connection with participation. I/We understand this consent to be valid for one year and may be withdrawn upon written notice. Upon written notice, I/we understand all information concerning the above-named player will be removed from the website. I/We further understand that if consent is withheld, this posting will not occur.
