PMHA Registration - 2024-2025

PAYMENT DUE DATES

Early bird payments must be paid by September 1, 2022, after this date the registration fee owed will return to the full amount.

ALL payments must be made by October 1, 2022.  No participant will be allowed on the ice if full payment is not received.

PAYMENT OPTIONS

I agree that full payment will be made by cheque, e-Transfer or cash. 

Cash Payments:  Please contact  Heather (Treasurer) at powassanhawkstreasurer@gmail.com to arrange transfer.

Cheque Payments:  Please make cheque out to Powassan MInor Hockey Assocaition and mail to: PMHA, PO Box 141, Powassan ON P0H 1Z0

For drop off please contact Heather.

e-Transfer Payments:  Please email to powassanhawkstreasurer@gmail.com.  Make password Hockey  Please include participant's name and date of birth in the memo/note section of the e-transfer.

REFUND

I agree to pay PMHA all fees required for basic registration as well as any further fees as

determined by any Representative Hockey Program as required.  I agree that there may be

circumstances when a refund will be requested. 

I agree that any request for refund will be made after reviewing the Refund Policy on the PMHA

website and filling out the applicable form for any refund request.

Refunds due to COVID will be pro-rated
