Norwich Minor Hockey 2024-2025 Registration

Thank you for registering with Norwich Minor Hockey Association!

Your player's registration is only considered complete with the reciept of;

-payment (either by cheque(s) mailed to - Norwich Minor Hockey Association, PO Box 467, Norwich, ON, N0J 1P0)

or etransfer to: treasurer@norwichminorhockey.ca (please put the player(s) name and birth year in the comments)

-payment is to be made at time of registration submission

-If you would like to discuss paying in installments (up to 4 equal post dated cheques, or 2 equal etransfers please contact treasurer@norwichminorhockey.ca

-copy of the player's birth certificate uploaded to player's HCR 3.0 profile under "documents" if they are new the Norwich Minor Hockey

-copy of a parent's driver's licence uploaded to player's HCR profile uploaded under "documents" as proof of residency

-reading and checking off all of the waivers on the player's HCR profile when registering

-$200 volunteer bond cheque (post dated for April 1st 2025), or a seperate etransfer of $200 with players name/birthdate and that the amount is for the volunteer bond

-completion of Respect in Sport Parent (if not already completed previously, although you will have to add your new player to your Respect in Sport Profile under "add member" even if completed prior for another participant
