PAYMENT: Full payment must be made by cash, cheque or e-transfer on or before September 30, 2024. Registration will not be complete until full payment is received.

Cheque Payment:&nbsp;make cheque payable to&nbsp;Mindemoya Thunder Minor Hockey Association and Contact Delray McCarthy, Treasurer via e-mail MTMHAFinancial@gmail.com&nbsp;to make payment arrangements. Please include player name(s) in the memo section of the cheque.
REFUNDS: Refund requests must be made in writing. Refund requests will be reviewed on a case-by-case basis at the discretion of Mindemoya Thunder Minor Hockey Association Executive. Registration fees will be non-refundable as of October 15, 2024.
EQUIPMENT:&nbsp;I/We, at the end of the season covered by this registration, agree to return all equipment provided by Mindemoya Thunder Minor Hockey Association, in good condition, and should we fail to do so we agree to reimburse Mindemoya Thunder Minor Hockey Association for the replacement cost of such equipment.
SIGNATURE &amp;&nbsp;WAIVER:&nbsp;I/We, hereby acknowledge the authority of&nbsp;Hockey Canada, Ontario Hockey Federation, Northern Ontario Hockey Association, Manitoulin Minor Hockey Association and Mindemoya Thunder Minor Hockey Association&nbsp;and agree to carry out and abide by the Constitution, Bylaws, and rules and regulations of those associations.

RELEASE: In consideration of this application to play under the auspices of Mindemoya Thunder Minor Hockey Association, I/We hereby for myself, heirs, executors, administrators and assigns, remise, release and forever discharge Hockey Canada, Ontario Hockey Federation, Northern Ontario Hockey Association, Manitoulin Minor Hockey Association and Mindemoya Thunder Minor Hockey Association, its officers, or anyone acting on their behalf from any manner of litigation, damage, claims, or demands in law or equity which I may have or acquire by reason of personal injury, loss or damage to property, which may occur during or by reason of participation in the activities of Mindemoya Thunder Minor Hockey Association.
