MILLET MINOR BALL PHOTO RELEASE FORM 
I hereby grant Millet Minor Ball permission to use my likeness in a photograph, video, or other digital media (&ldquo;photo&rdquo;) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Millet Minor Ball and will not be returned. I hereby irrevocably authorize the Millet Minor Ball to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Millet Minor Ball from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Consent to Play By submitting this registration form, the parent or legal guardian has given their consent for this child to participate in the athletic activities as a member of Millet Minor Ball. It is further more understood and agreed that all managers, coaches, umpires, officials and sponsors will not be held responsible for any injury or damage incurred before, during or after the event. Further consent is given for the medical treatment, if required, for injury or illness as a result of this participation. In the absence of the parent or legal guardian, the team coach is given consent to sign the medical treatment form.
Equipment Requirements
Each player is responsible for their own jock or jill. This is a mandatory item for player safety. Players are responsible for this own black baseball pants, baseball glove, plastic/rubber cleats and batting helmets. Please ensure that your player's equipment is labeled so it is not lost or taken by mistake. Millet Minor Ball will provide a jersey and ball cap for all players for use during their season. This must be returned at the end of the season or the UNIFORM DEPOSIT of $30.00 will be not be refunded. 
Volunteer Requirements All registering families are required to work 8 hours/family during the 2024 season. This may include concession, 50/50, field maintenance, and event set-up/take-down during tournaments, double header days, etc. Once tasks have been assigned, volunteers are solely responsible for completing tasks. Any switching between families must be communicated directly to the event coordinator (not coaches or team managers). You are responsible for your original assignment until completed. Bottle Drive fundraiser does not count towards volunteer time. Only individuals over 16 years of age are allowed to perform volunteer tasks. Payment Arrangements Please ensure all payments are made prior to the start of the playing season.
Email &amp; Text Use I agree to allow Millet Minor Ball to contact me at any of the phone numbers and/or email addresses listed in player registration. This can include but is not limited to game times and changes, volunteer information, upcoming events, team communications, coach requests, etc. Use of other apps for communication is solely used at the coaches discretion and is the not the responsibility of Millet Minor Ball.
Team Limit Note: for the 2024 season the first 12 paid registrants will be guaranteed team spots. The final 3 registrants will be considered wait list.&nbsp;
