This registration, when completed, confirms the player's commitment to accepting a position on the Markham Waxers team to which they are registering. I understand, that having commited to this team, I can no longer attend evaluations for any other OHF team.
All Markham Waxers participants and their families agree to abide by all Hockey Canada, OHF, OMHA, ETA, YSMHL, and Markham Waxers policies, by-laws, rules, regulations and codes of conduct.
