2024-2025 PARKWOODS PLAYER REGISTRATION

Payments can be made in one payment by e-transfer or credit card via our Parkwoods Hockey website. Please note that payments by credit card are subject to additional fees from third-party providers. Please ensure you receive confirmation from Hockey Canada that your profile is complete. If, after registration, you receive an email from Hockey Canada indicating that anything has been rejected or is missing, please login to your Hockey Canada account to address deficiencies. 

It is understood that Parkwoods Hockey League ("Parkwoods"), its Executive and Officers, are not responsible for injuries to players, spectators, or officials and that Parkwoods would not accept the above named child into the league WITHOUT this waiver of responsibility.

I accept the above conditions and agree to indemnify Parkwoods, its Executive, Officers, Coaches and Volunteers against any damages or costs that may arise as a result of any claim brought on either my behalf or my child's behalf against them.

I shall ensure that the said player obeys all of Parkwoods' rules and regulations and further acknowledge that the player named herein is in good health and is able to participate in hockey. I also hereby release Parkwoods from all claims for damages arising from any accident or injury which is caused by or arises from the participation in the sport of hockey by the player named herein.

I FULLY UNDERSTAND AND ACKNOWLEDGE THAT THE PLAYER MAY PLAY FOR ONLY ONE ORGANIZATION AND THAT THE PLAYER MUST OBTAIN A RELEASE FROM THE HOUSE LEAGUE PRESIDENT IF HE/SHE WISHES TO PLAY FOR ANOTHER ORGANIZATION.

No release can be given after November 15th. House league player insurance protection shall commence from either Sept. 1 of the current season or on the date that the player and the appropriate form is registered with Parkwoods' governing body, The Greater Toronto Hockey League ("the GTHL") office, whichever is the later. Final registration date is January 15th.

Privacy Statement: The information requested on this form is required by the GTHL (its employees, coaches, volunteers and member associations) to process the player's registration form and to administer the rules of the GTHL House League. It is also used to provide notice of GTHL events, as appropriate.

The GTHL may, if required, request proof of a player's identity and date of birth. From time to time, the GTHL may use or disclose to third parties, the player personal information collected on this form, for the purpose of offering additional products and services, including promotional items, which may be of interest to the player.

The consent of the player, or the player's parent or legal guardian, for the use of player personal information for the purposes of offering additional products and services, including promotional items, is optional.
