DOFASCO MINOR HOCKEY LEAGUE

Rules of Conduct, E-mail Communication, Information Use Policy

I agree to abide by all rules, regulations and by-laws of the DMHL, in this or any successive years of participation, including ArcelorMittal Dofasco Code of Behavior and ArcelorMittal Dofasco Zero Tolerance Policy, By registering with DMHL, the applicant and their parents or guardians acknowledge that they are opting-in to receive email communications from DMHL and that any and all email communications from DMHL to them does NOT constitute SPAM or unwanted email communications.

I further understand that the DMHL collects information about myself, child/children and other family members or child guardian such as names, home address, email addresses, telephone numbers, banking information or image and will consider it personal and confidential not already available publicly. I agree that DMHL can use and disclose the provided personal information for purposes of registration, administration of DMHL, Alliance Hockey, OHF and Hockey Canada rules and regulations, publishing player, game or event information in such form or deemed as it appropriate to promoting the results and performance of any of the individual player, teams, or divisions or the Association as a whole.

Photo, Video, and Image Release

I hereby grant ArcelorMittal Dofasco Minor Hockey League  ("DMHL") the irrevocable right to use or license others to use worldwide, without compensation, my image or likeness captured during any game, tournament, event or activity in any photographic, video, digital imaging or other audio/visual format for the purposes of publicity, marketing, promotion, news clips, public service announcements, video productions, television broadcasts, and/or website postings during the current hockey season. I also understand that my name may be used in conjunction with such images.

Indemnity, Authorization and Acceptance Policy

I acknowledge and accept on behalf of myself, son/daughter and other family members, that the named participant may be moved within the House League system for the purpose of equalizing teams. It is agreed that any movement may take place after any initial league or divisional draft and be initiated at the discretion of the DMHL.

 I understand that my child/children participate(s) in these activities at their own risk and hereby release each of ArcelorMittal Dofasco G.P., F.H.  Sherman Recreation & Learning Center, and the Dofasco Minor Hockey League from any claims of any kind for any damage to any of our property or injury to my child/children.

 I understand that my child/children playing privileges can be revoked immediately, at any time, at ArcelorMittal Dofasco’s sole discretion

I understand I am responsible for any damages I or my child/children my cause to facilities and I understand that I will be billed for the repairs

 

DOFASCO MINOR HOCKEY LEAGUE

Refund Policy

As per the Dofasco Minor Hockey Constitution Article #11 the refund Policy States:

1. In the event of a late registraion the fee will not be prorated
2. A $75 non-refundable fee will be levied on all refunds
3. Refund Schedule - Non Goalies
   1. 75% refund after September 30
   2. 50% refund after October 31
   3. 25% refund after November 30
   4. 0% refund after December 31
4. Refund Schedule -  Goalies *
   1. $150 Charge until November 30. (Goalie Fee - $150 = refund)
   2. %25 refund after November 30
   3. 0% refund after December 31
5. Refunds can be modified on a case by case basis by the Steering Committee.
