<h2><strong>Terms and Conditions </strong></h2>
<p>All Hockey Canada, OHF, OMHA, CDMHA and Haldimand County rules and conditions must be adhered to by all players and parents/guardians. While attending any hockey games, practices or any hockey related event, all players, parents/guardians are expected to act in a respectful manner.&nbsp;I understand that failure to comply with this may result in disciplinary action<b></b></p>
<p>All registration payments must be paid in full, or installments arranged prior to participation in any on-ice activity.</p>
<h3>Payments&nbsp;can be made by:&nbsp;</h3>
<ul>
<li>credit card at the time of online registration OR</li>
<li>E-transfer to&nbsp;<strong><em>etransfer@cayugaminorhockey.ca</em></strong> &ndash; use<strong> &ldquo;Cayuga&rdquo; </strong>for security question answer.<strong> </strong><strong>** Be sure to include Player name(s) and their Division(s) in the comments box.&nbsp;</strong></li>
</ul>
<h3>Refund Policy:</h3>
<p>If a player terminates participation of their own volition, after the season starts (first CDMHA ice rental) via a written request to the Registrar, their registration fee will be refunded minus a $20 Administration fee as follows:</p>
<ul>
<li>On or before October 31 - 75%</li>
<li>From November 1 to November 30 - 50%</li>
<li>From December 1 to December 31 - 25%</li>
<li>No refund will be given after December 31 unless otherwise approved by the Board upon consideration of special circumstances.</li>
<li>Players who are asked to leave the CDMHA by their conduct on or off the ice will receive no refund.</li>
<li>Refunds can be obtained upon the return of any equipment belonging to the CDMHA in the same condition that it was issued.</li>
