Please complete the next step and go to the Ontario Hockey Federation (OHF) website for the Screening Submission Portal. 

You will be required to have your HCR number available.  This number is available on your spordle.com/account page.

Submission of a valid (within 3 years) Vulnerable Sector Check must be on record with OHF.  IF this has been completed, please continue to the declaration page (must be completed annually)
