Terms and Conditions
By completing this registration, I agree to abide by the rules and regulations of South Grey Minor Hockey Association (SGMHA), its governing bodies and any affiliated leagues and tournaments.
I hereby release SGMHA, all Coaches, Bench staff and Officers from all claims of damages resulting from any accident which may arise from participation in SGMHA activities.
No players will be permitted on the ice until his/her/their registration is completed and the applicable fees are received by SGMHA.
A completed registration and full payment does not guarantee a player placement on a team as space may be limited by the number of teams and/or team sizes in any given season.
New Players
A copy of a new players birth certificate must be provided to SGMHA. One parent/guardian/caretaker per household must complete the Respect in Sport Parent course. Your player(s) will not be able to be rostered without a completed course. OMHA my also request proof of residence.
