_{I shall see that the said player will obey all club rules and regulations, and that, the player named heron is in good health and is able to participate in hockey. I also hereby release the hockey club and organization from all claims for damages arising from any accident or injury which is caused by, or arises from participation in the sport by the person named hereon as the player. I REALIZE THAT THE PLAYER MAY PLAY FOR ONLY ONE ORGANIZATION AND MUST OBTAIN A RELEASE FROM THE HOUSE LEAGUE PROGRAMS MANAGER IF HE/SHE WISHES TO PLAY FOR ANOTHER ORGANIZATION. No release can be given after November 15. House league player protection shall commence Sept. 1 of the current season or on the date of the registration of the form with the G.T.H.L. office, whichever is later. Final registration date is Jan. 15th.}

_{The undersigned does permit and authorize the Streetsville Hockey League (“the SHL”) and its employees, agents, and personnel who are acting on the organization’s behalf to use the photograph or other likeness of the person named herein for purposes related to the mission of the organization, including publicity, marketing, and promotion of the organization and its various programs. I understand that the photograph or likeness may be copied and distributed by means of various media, including video presentations, television, news bulletins, mailouts, billboards or signs, brochures, placement on the organization’s websites, or newspapers. I understand that, although the organization will endeavour  to use the photograph or likeness in accordance with standards of good judgment, the organization cannot warranty or guarantee that any further dissemination of the photograph or likeness will be subject to the organization’s supervision or control. Accordingly, I release the organization from any and all liability related to dissemination of the photograph or likeness. I have read this document and understand its contents.}

_{Registration fees are refundable up to June 1st of the upcoming season, subject to the administration fee below. From June 2nd to July 1st a $100.00 surcharge will be deducted from the refund. From July 2 to August 2 a $150.00 surcharge will be deducted from the refund. No refunds will be given after August 3.}

_{No refund request shall be processed unless submitted in writing to the Programs Manager either as a hardcopy letter signed by the person requesting the refund, or through e-mail.}

_{No refund cheques will be issued until all league equipment has been returned in good repair and it has been inspected by the Programs Manager that such is the case. All refund requests are subject to a 4 to 6 week processing period.}
