All refunds are pro-rated as follows:

Advising a coach, convenor or any other person at Chedoke Minor Hockey of your wish to withdraw is not considered a formal refund request and will not be processed as such. 2. The date used to determine the refund amount is the date this form is received. 3. Additional fees may be withheld if uniforms have been given out and are not returned. 4. All refunds will be issued in the form of payment, credit card, cheque etc. 5. Please allow 3 to 4 weeks for processing. 6. Refunds due to injuries received during play with CMHL will be assessed on an individual basis. A doctor's note may be required.
