BWI Summer Season (Juniors/21U)

I have read and accept the following:

• I CONFIRM THAT MY CHILD IS A RESIDENT OF DORVAL OR POINTE-CLAIRE OR HAS PLAYED FOR BWI LAST SEASON. BWI WILL CANCEL ANY REGISTRATIONS THAT DO NOT MATCH THESE CRITERIA.

Cancellations and Refunds

• Cancellation: Prior to June 1: 100% refund. There is no refund after this date.
• Injury Refund: Prior to June 1: 100% refund. After June 1, 50% refund as long as the registrant missed 50% of the scheduled games and can provide a medical note.
